We are grateful to have received a grant from the Comox Valley Community Foundation to establish a safe, stable housing fund.
Vulnerable members in our community often require healthcare support in the community in order to leave the hospital knowing their care needs will be met. In planning for at-home care, a home visit is completed by an Island Health Community Health Services team member. Sometimes during the visit, staff may discover a home is not safe for the patient to live in or for healthcare workers to enter. Reasons can include hoarding, house maintenance concerns, or the lack of physical supports.
Comox Valley Community Health Services provides between 800 to 1,000 home support visits per day 365 days a year. The grant from the Comox Valley Community Foundation has provided funds for the CHS team to draw upon to help ensure a person’s home will provide a safe place to return to and to receive ongoing care.
