A grant toward the Transition Suites Food Program has been generously provided by the Comox Valley Community Foundation.

Ensuring the highest level of care for members of our community is vital and the generosity of the Comox Valley Community Foundation is furthering this objective. Vulnerable community members who are transitioning from hospital acute care to long-term care or assisted living will be provided nutritious meals that will contribute to their well-being and continued care.

The Transition Suites at the Washington Apartments are an important service offered by Community Health Services that can provide live-in overnight services and up to six visits a day to support clients at a long-term care level. The eight suites ensure patients receive the best care in a safe, comfortable place to ensure a smooth transition. This funding support will in turn contribute to more efficient use of acute care resources at the Comox Valley Hospital.
